**To get records released from Great Smokies Diagnostic Labs, Please notify our lab (lab@wholisticpeds.com). Dr Berger was the ordering physician so they will release records directly to our office
Urine neurotransmitter tests sent to Neuroscience Labs that were ordered under my previous practice must be obtained from the old office. The laboratory will not release these results to you or me.

Dear Friends,

For those families who have a chart in another practice and need them transferred to me, I am developing a system to make this as seamless and easy as possible. Since most families have most of their records already, it is not necessary to have an entire chart copied and sent. Rather, families should request only the records that they are presently lacking.

IMPORTANT INFORMATION
HIPAA (the laws that protect your rights to access your medical records) entitles the patient or responsible person to access to the patient's protected health information in the form or format requested by the individual, especially if it is readily available in that format. Please request your patient records using the records transfer form provided below (see link), or the form used by your previous practice, and request that the information be forwarded by email to you, if that is the format in which you want your records sent.
An emailed format is the preferred way to transfer records to my practice.
IMPORTANT NOTE: My former practice will only accept their own records release form. It is available on their webpage or by calling that office.

Please remember that most medical offices are very busy, and it make take time for them to fill your request. They would appreciate your patience.

The following steps are necessary to make this work correctly:



1st: Register and Authorize:

Even if you were a prior patient, you must register with Wholistic Pediatrics, we are new, and so are you! :) WELCOME!. Please download and fill out the
registration and authorization forms
2nd: Obtain your Medical Records and Lab data by following the instructions above.
3rd: Send us the records
To send us the files you can do any of these 3 things:
1) SCAN: You can scan records into your computer and e-mail them. The format/type of file is not of importance at this time. To save you worries and time, you can scan your files all as one document and named as below. Know that if you can open the file on your computer, I should be able to also.

2) If you do not have a scanner you can transfer records onto CD and e-mail them, or snail-mail the disks. Kinkos or a good copy service can do this. Even the library can be of aid.

3) FAX: We are now able to receive faxes of medical records. Please be aware that less success is had with this option, since we can only accept 1 incoming fax at a time, and this will be competing with our outgoing faxes. There can be busy signals and delays.

Why all this bother? We need the records to be in a digital format, since that is what is being put in the chart. It would be impossible for us to scan tens of thousands of pages if everyone sends hard copies.


From all of this you can see why it is preferred that you e-mail us the records.
NOTE: CHANGE OF POLICY
You can send the file all together at once, or “batch file” your documents and name them with your child(ren)s’ name as such:

LASTNAME.First Name
ALLCAPS.Case Sensitive
LASTNAME-SECONDCHILD.First Name
DOE.John
DOE.Jane

Chronological Order is a MUST with Earliest Date being page 1. Don’t worry if it is a “Consultation” note or Lab data. Keep the dates in order. The most recent results and office notes are the most valuable, but records from years ago are too... Do not leave those out!

Another savings in time is to send only what is necessary. For example:
~ We do not need papers that the laboratory provided as interpretation or discussion of the results. That is for your education.
~ We do not need any pages from a laboratory that do not contain specific results. Sometimes there are blank pages from the laboratory, or pages that just have their address.
~ when requesting your records, ask that the same courtesy be extended to you.
We fully understand all the intensive work involved in getting this process completed. As you have one patient record to process, we have many and your contribution and abilities are very, very, much appreciated. You are not only helping your child or children to have continuity of care, but you are doing a great service for our Future Generation by saving paper (trees)!!! *** For that, we can all feel good .
